I've brought numerous dogs on the plane. You need a health certificate from a vet within 5 days of the flight. I always tell the crew on the plane that I'm flying with my dog and will be waiting for the tag that says your dog is on board the plane. I tell them I'm ready to get off if the dog doesn't get on. For my 25 # nervous dog, the vet recommended 1/2 to a full tab of gravol. They cannot overdose and just get sleepy. Much success with the pill as without it she looks like she's been shaking the whole time. My golden lab doesn't need anything as he is mellow about everything anyways.
On the customs forms you need to declare that you're carrying a live animal.
