The application form is available on INM's web site, http://www.inm.gob.mx. The form is in Spanish, but the information pages are available in English. Although you can submit the application on line, you still have to make two trips to Manzanillo: once with all your supporting documentation (passport, FM booklet, bank statements, proof of residence, etc.) and again to pick up your new card.
