I just went 10 days ago. I was prepared with infantantil color photo front facing with hair back and no adornments, copies of three months bank statements, a copy of pages 1 and 2 of my passport,a copy of my phone bill with my address on it and finally with my expiring 5 year old FM3. I did go to the website and found the form. I filled out the information on line with help from a friend on the internet and pushed the send button, a receipt number appeared on the screen. I copied it down. I was unable to print the form from the website. When I arrived at the IMSS office they asked for my receipt number, they looked it up, had me answer some questions describing scars and features, looked at my documentation, kept the picture of me, my passport, proof of residence, expiring FM3 and everything else was returned to me. I was asked to go to the bank and pay 1495 pesos and when I returned I was given a receipt for my old FM3 and told to come back in two weeks for my new photo ID card containing a magnetic strip. It was easy but I have heard of people doing it all in the same day. Not sure how or what they did differently maybe it their FM3 was not expiring, but I will return to pick it up this week. It seems easier and faster. I hope this helps.
