Hi Tia: When the item was important (i.e. prescription glasses) we used UPS and it was delivered right to my door using my name, street address, La Manz, Jalisco, Mexico and postal code. We have friends who did not know we have a letter box in Melaque and had sent us some letters and cards addressed as above and we have received this mail - delivered straight to our home! Some generous person collected our mail and took it to the CFE person so we would get it and another generous person who knew us brought it to our home! So, in answer to your question, use your first example of address form if you do not have a letter box in Melaque.
