Dave, I have spoken to Dr. Itzel, it is acceptable to give her a tip, she has been told this by her boss in La Huerta. She cannot charge for her services to gringos (or the locals), that is what she could get fired for. The ladies here do not run the clinic, and Dr. Itzel answers only to her director in La Huerta. Yes, some of the locals will raise a fuss over her servicing gringos whether they tip or not. Trust me on that one, I speak from experience. My suggestion is that if you want to tip the lady, ask her about it when you have a consultation with her. I am not trying to be arguemenative on this at all, I am just repeating what Dr. Itzel herself has told me.
