We just renewed our FM3 in Manzanillo, and they asked for 4 black and white photos: 2 front shots, and 2 "profile" shots. They also wanted bank statements for the most recent 3 months showing a mininum balance of 13,000 pesos per month, and a current electricity bill from our domicile in Mexico. The fee had to be paid at a bank nearby to proceed - i think it was around 1,500 pesos. We were told to return a month later to get the actual visa.
