I'm not an expert, but in my 8-year old house none of my tiles have failed, even those outside exposed to the elements. My speculation:

1. Non-ceramic Mexican floor tiles are somewhat porous and vulnerable to moisture. I have seen quite a few that have lifted. If you really want Mexican tile, there are high fired ones available at a higher price.b

2. Improper concrete subfloor, which should certainly not be powdery. Some builders add too much additional lime to the mix, okay for flexible stucco walls but not in floors and foundations.

3. Improper floor adhesive used. Use a good quality floor adhesive, not a wall tile adhersive or anything else.

4. Uneven subfloor, which could allow air gaps under tiles.
