As a family practice physician, I carry a things I may personally need medically, so I appreciate your concern and precautions. My husband and I have spent time familiarizing ourselves with local medical care--not always an easy task and it does change. Since we have no ambulance in La Manzanilla and emergency care is minimal, it is a good idea for all of us to be familiar with what to do when an emergency arises.
You are very wise to carry antihistamines and Epi Pen. They HAVE BEEN used effectively for scoprpions here. Many people take antihistamines right away or do nothing and are OK.
A couple years ago my husband had a scorpion sting on his finger about 10pm. Since I am a physician, I decided to keep him at home after taking Benedryl. I had him lay in the van all night so we would be ready to go to Marta(the pharmacist/dr. in La Manz.) for antivenom (10 minute drive)at anytime that night. Luckily, pain was his biggest problem. Ice and medication helped with that and he slept while I monitored his breathing, etc. during the night.
The closest hospital is in Melaque--a 30-40 minute drive over a 2 lane mountain hwy.which is not fun at night. This is a small public hospital and you may want to visit it so you know where it is located and what services they can offer you. They can transfer you to a larger hospital by ambulance if necessary.
We once had a nicely equipped ambulance in La Manz. but no more. Visiting Marta and the local clinic in La Manz. will be important for you. Emergency services are sparse here.
